Динамика когнитивных функций и социальная адаптация
у пациентов с шизотипическим расстройством (ШТР)

Аннотация. Цель исследования состоит в изучении возможного влияния когнитивных нарушений у пациентов с ШТР подросткового и юношеского возраста на их социальную адаптацию. Материалы и методы. Основная экспериментальная группа — юноши, больные шизотипическим расстройством, находящиеся на стационарном или амбулаторном наблюдении в ФГБУ НЦПЗ РАМН, 33 чел., в возрасте 17—28 лет, средний возраст 22 года. Из 33 пациентов 12 наблюдались в НЦПЗ РАМН, начиная с подросткового возраста (12—15 лет), катамнез от 3-х до 12 лет, среди них был выделены группа ремиссии (ГР) и группа с неблагоприятным течением заболевания (ТЗ). В контрольную группу вошли здоровые испытуемые, отобранные с помощью опросника SPQ, 20 чел., студенты и выпускники МГУ, мужского пола, в возрасте 17—28 лет, средний возраст 20 лет. Использовались методики: «Заучивание 10 слов», «Таблицы Шульте», «Малая предметная классификация», «Конструи-рование объекта, «Шкальная самооценка» (Дембо — Рубинштейн), также проводились клиническая беседа, изучение медицинской документации, беседы с родственниками. Результаты исследования и обсуждение. Сравнение группы юношей с ШТР и контрольной группы показало, что юноши с ШТР имеют более низкий социально-экономический статус (у них больше академических отпусков, 21% испытуемых перешли на обучение в менее престижные учебные заведения, 36% испытуемых не учились и не работали). Результаты исследования показали, что в подростковом возрасте группы ремиссии и текущего заболевания практически не отличались между собой по когнитивным показателям и поэтому они не являются предикторами успешной адаптации в юношеский период. В юношеском возрасте отмечена положительная динамика по показателям памяти в обеих группах, важно отметить, что в группе ремиссии некоторые показатели достигали нормативного уровня (в среднем они запоминали 7 слов при непосредственном воспроизведении). Показатели внимания практически не отличались от таковых в подростковом возрасте, в юношеском возрасте показатели внимания не имели положительной динамики и по сравнению с контрольной группой пациенты отыскивали числа достоверно медленнее (Р≤0,01). Показатели мышления (уровень обобщения) в юношеском возрасте группы ремиссии имели положительную динамику, при этом в целом показатели мышления достоверно не отличали всю группу ШТР от группы контроля, поэтому только показатели памяти в юношеском возрасте являются более четкими предикторами дальнейшего прогноза. Анализ социального статуса показал, что адаптация группы ремиссии (ГР) значительно выше, чем в группе текущего заболевания (ТЗ). Пациенты ГР старались занимать активную социальную позицию, практически все имели друзей, работали или учились. В группе текущего заболевания (ТЗ) почти половина пациентов, несмотря на полученное образование, не работали, основное время собирались проводить дома, т.е динамика их социальной адаптации была отрицательной. Заключение. Таким образом, проведенное исследование показало, что именно личностные особенности (низкая самооценка, снижение энергетического потенциала, эмоциональное уплощение, волевые нарушения и социальная изоляция) играют основную роль в социальной дезадаптации пациентов с ШТР, тогда как когнитивные нарушения в юношеском и особенно подростковом возрасте не могут быть использованы в качестве четких коррелят адаптационных возможностей.

Введение

Шизотипическое расстройство является довольно распространенным психическим заболеванием, так по данным Американской психиатрической ассоциации встречается до 3—6% случаев заболеваемости [17; 18]. В нашем предыдущем исследовании Кобзова и др. [9; 10] в 3% случаев среди когорты здоровых испытуемых был выявлен высокий риск развития ШТР. Несмотря на неоднозначную трактовку данного заболевания [2; 15; 27] отечественные и западные исследования показали, что шизотипическое расстройство может сопровождаться когнитивными и личностными нарушениями, социальной дезадаптацией [1; 5; 6; 11; 12; 13; 14; 21; 23; 24; 26; 35], связано преимущественно с негативной симптоматикой [3; 19] и в 50% случаев больные с шизотипическим расстройством являются родственниками первой степени родства больных шизофренией [28]. При этом дефицит в когнитивной и личностной сфере у больных ШТР может являться предиктором шизофрении [20], в связи с этим важной задачей является изучение динамики различных аспектов функционирования больных с заболеваниями шизофренического спектра, а так же выявление предикторов этих заболеваний. Исследованию динамики клинических и психологических особенностей у пациентов с заболеваниями шизофренического спектра, а также анализу видам динамики (терапевтической, катамнестической, возрастной) посвящены работы таких авторов, как В.Г. Каледа [8], Критская В.П. [12], Мелешеко Т.К. [13], Борисова Д.Ю. [1], Хромов А.И. [16], A. Rainе [22; 33], Ledda [25].

Исследование показателей памяти и их динамики. В работе В.П. Критской, Т.Д. Савиной [11], было показано, что шизофренический процесс сопровождается нарушениями памяти — изменением структуры мнестической деятельности, нарастающими по мере углубления дефекта. У пациентов с ШТР Heaton и др. [31] отмечали дефицит запоминания в начале выполнения задания, при этом отсроченное воспроизведение информации не отличалось от контрольной группы (нормы) Bergman и др. [35], другими исследователями отмечается нарушение проспективной памяти (памяти на намерение) у больных шизотипическим расстройством [32].

В работе Хромова А.И. [16] изучалась динамика непосредственной слухоречевой памяти у детей (7—8 лет) и подростков (14—16 лет) в группах шизофрении, шизотипического расстройства и группы здоровых. Было показано, что отставание от нормы по объему непосредственного воспроизведения заканчивается 15—16 годам в группе пациентов с ШТР. У них было отмечено увеличение объема памяти до возрастной нормы.

Исследование показателей внимания и их динамики. Среди нарушений внимания у больных расстройствами шизофренического спектра авторы выделяют: дефицит обработки и приема информации [29], неспособность больных шизофренией выделять главный стимул [30], нарушение активного внимания, связанного с аффективной сферой и мотивацией и пассивного (селективного) внимания. Для пациентов с ШТР также был характерен более длительный кросс-модальный перенос от визуального к слуховому раздражителю [36]. В исследовании А.И. Хромова [16] было показано, что возрастные изменения внимания у больных с расстройствами шизофренического спектра, так же как и в норме, демонстрируют отчетливую положительную динамику, отставание от нормы в группе f 21, выраженное в возрасте 7—14 лет, нивелируется к 15—16 годам.

Изучение социальной дезадаптации и ее динамики. Катамнестическое исследование (5 лет) Ledda и др. [25] подростков с диагнозами шизофрения, шизоаффективный психоз и биполярное расстройство показало худшую динамику социальных и когнитивных параметров в возрасте 20—25 лет в группе у пациентов с шизоаффективным психозом и шизофренией в отличие от пациентов с аффективным заболеванием.

Таким образом, краткий обзор отечественных и зарубежных работ по изучаемой проблеме показал, что изучение динамики при заболеваниях шизофренического спектра является актуальной задачей, стоящей перед медицинскими психологами, клиницистами, психофизиологами. При этом работ, посвященных катамнестическим исследованиям пациентов с шизотипческими расстройствами в возрастном интервале от 12 до 28 лет практически нет. Поэтому цель настоящего исследования состоит в изучении возможного влияния когнитивных нарушений у пациентов с ШТР подросткового и юношеского возраста на их социальную адаптацию.

Материалы и методы

В исследовании участвовали подростки и юноши с шизотипическим расстройством (F21.х согласно МКБ-10), а также здоровые испытуемые в возрасте от 17 до 28 лет. Все здоровые испытуемые (20 студентов и выпускников МГУ, средний возраст 20 лет), были отобраны на основании тестирования с помощью опросника SPQ (Schizotypal Personality Questionnaire) (А. Rainе [33], Ефремов А.Г., Ениколопов С.Н. [4]). Только те испытуемые, у которых суммарный балл по этому опроснику был ниже 23, были включены в эту группу. Критерии включения в клиническую группу: возраст от 17 до 28 лет, мужской пол, наличие диагноза: «шизотипическое расстройство», начало заболевания в подростковом возрасте. Основная экспериментальная группа больных ШТР (33 человека, в возрасте 17—28 лет, средний возраст 22 года) находилась на стационарном лечении, либо на амбулаторном наблюдении в НЦПЗ РАМН. Все больные принимали психотропную терапию и были обследованы в состоянии выхода в ремиссию. Из 33 пациентов 12 человек наблюдались в НЦПЗ РАМН, начиная с подросткового возраста (12—15 лет), катамнез от 3-х до 12 лет. 5 человек находились в стойкой ремиссии (группа ремиссии (ГР), тогда как другая группа (7 человек) были стационированы в НЦПЗ РАМН повторно в юношеском возрасте (группа с текущим заболеванием (ТЗ)). Все пациенты были диагностированы врачами-психиатрами.

Исследование когнитивных функций. Для исследования показателей памяти и внимания применялись методики «Заучивание 10 слов» и «Таблицы Шульте», анализировались объем слухоречевой памяти (количество запоминаемых слов), объем внимания (время, затрачиваемое на отыскивание чисел в таблицах Шульте). Мышление изучалось с помощью тестов «Малая предметная классификация» и «Конструирование объекта», сравнивались между группами такие показатели, как уровень обобщения (способность испытуемого оперировать категориями), нарушения мышления (классификация групп с опорой на конкретные признаки, с искажением обобщения, с созданием ситуаций и комплексов), коэффициент стандартности (оценивается насколько стандартно или нестандартно испытуемые выполняют задание, на каких общепринятых свойствах предметов основаны их заключения) и продуктивность (количество выполненных заданий). Кроме того, исследовалась самооценка пациентов и испытуемых контрольной группы с помощью методики «Шкальная самооценка» (Дембо — Рубинштейн) по параметрам: «здоровье», «ум», «счастье», «характер».

Исследование социальной адаптации проводилось с помощью клинической беседы, изучения медицинской документации, бесед с родственниками.

Математическая обработка данных проводилась с помощью программ Statistica 7 и приложения MS Office Excel. Для оценки результатов, полученных в катамнестических группах, из-за небольшого числа пациентов входящих в эти группы, в индивидуальных случаях (в группах катамнеза) применялись и анализировались дополнительные патопсихологические и личностные опросники. Мы использовали количественный анализ средних, а также качественный анализ. Для сравнения данных общей группы ШТР и контрольной группы применялся t-критерий Стьюдента, достоверными являлись различия при Р<0,05.

Результаты исследования

Сравнительный анализ социально-экономического статуса пациентов с ШТР и контрольной группы в юношеском возрасте

Социальный статус экспериментальной группы был ниже, чем в контрольной группе по большинству параметров. У пациентов с ШТР больше академических отпусков, 21% из них перешли в менее престижные учебные заведения, 36% пациентов на время обследования не работали и не учились. Юноши из основной экспериментальной группы жили с родителями, тогда как в контрольной группе 30% юношей жили самостоятельно. Анализ результатов исследования двух катамнестических групп — группы ремиссии (ГР) и группы с текущим заболеванием (ТЗ) — показал, что уровень социальной адаптации в группе ремиссии значительно выше, чем в группе текущего заболевания, поскольку большинство юношей ГР работали, либо совмещали работу с учебой, а в группе ТЗ никто не работал, несмотря на полученное образование.

Для того, чтобы выявить связь между успешностью социальной адаптации и когнитивными нарушениями, мы провели сравнительное исследование показателей памяти и внимания у пациентов с ШТР юношеского возраста и контрольной группы, а также отдельно проанализировали эти показатели у пациентов ГР и ТЗ в подростковом и юношеском возрасте. Были обнаружены лучшие показатели памяти и внимания в контрольной группе. Все показатели методики «Заучивание 10 слов» были у них выше, чем в группе ШТР, однако достоверные различия получены только для параметров «непосредственное воспроизведение» (Р<0,04) и «второе воспроизведение» (Р<0,03).

Было интересно выяснить, являлся ли этот показатель в подростковом возрасте предиктором плохого выхода. Оказалось, что при первом обследовании в возрасте 12—15лет подростки из ГР воспроизводили слов меньше, чем из группы с ТЗ, т.е. этот показатель не может определить качество выхода. Более того, отсроченное воспроизведение у юношей из группы ТЗ было лучше, чем таковое в группе ремиссии.

Таким образом, было выявлено наличие положительной динамики показателей памяти, в частности, непосредственного воспроизведения в группе ремиссии. Однако, как показало проведенное исследование, показатели памяти в подростковом возрасте не являются предикторами успешности социальной адаптации.

Сравнительное исследование показателей внимания у пациентов с ШТР юношеского возраста и контрольной группы. Для исследования внимания применялась методика отыскивания чисел по таблицам Шульте. Исследование внимания показало, что группа нормы в среднем быстрее отыскивает числа по всем таблицам Шульте (33—35 сек. на таблицу) по сравнению с группой пациентов с ШТР (40—50 сек. на таблицу). Результаты достоверно различаются (p≤0.01). Сравнение средних показателей внимания по таблицам Шульте в группах ТЗ и ГР в подростковом и юношеском возрасте и юношей контрольной группы представлены на рис. 2.

На представленных гистограммах видно, что подростки из группы ТЗ быстрее находят числа, чем подростки из группы ГР, хотя достоверных различий не обнаружено. В группах подростков время отыскания чисел увеличивается к третьей таблице, что можно рассматривать как отражение процесса утомления. В юношеском возрасте у пациентов с ШТР этот показатель существенно не изменился. В обеих группах результаты отличаются от группы контроля, пациенты медленнее отыскивают числа, (39 сек. — минимальное время поиска в юношеском возрасте; 47,6 сек. — максимальное). В группе контроля (34,3 сек. — минимальное время поиска и 36,7 — максимальное время поиска чисел).

В целом можно сказать, что у пациентов с ШТР показатели внимания оказались хуже, чем у испытуемых контрольной группы, при этом не наблюдалось отчетливой возрастной динамики показателей внимания в обеих группах (ремиссии и текущего заболевания) и не отмечено разницы в этих показателях у пациентов ГР и ТЗ. Т.о., показатели внимания также не явились предикторами успешности социальной адаптации.

Сравнительное исследование мышления у пациентов с ШТР и контрольной группы. Сравнение параметров мышления: «уровень обобщения» (ОБ) и «нарушения мышления» (НМ) в группе пациентов с ШТР и в контрольной группе показало, что в группе ШТР уровень обобщения (58%), практически идентичен группе контроля (59%). Оценивая возрастную динамику по показателю обобщения в двух группах, можно отметить положительную динамику в группе ремиссии и отсутствие таковой в группе ТЗ. Т.о., показатель «уровень обобщения» в подростковом возрасте не может предсказывать дальнейшую динамику состояния.

Уровень нарушений мышления у лиц юношеского возраста оказался ниже в группе ШТР (29%), чем в группе нормы (39%), однако достоверных различий не выявлено.

Сравнение группы ШТР и контрольной группы в тесте «Конструирование объекта». Сравнение коэффициентов стандартности в тесте «Конструирование объекта» в группе пациентов с ШТР юношеского возраста и контрольной группе показало, что коэффициент стандартности выше в группе контроля (0,63), чем в группе ШТР (0,58), однако эти данные достоверно не отличаются. Кроме коэффициента стандартности мы сравнивали группы по продуктивности. Продуктивность одинакова в обеих группах: 0,88 и 0,85 балла.

Т.о., показатели мышления не выявили существенных различий между группами ШТР и контрольной группой, что также не позволяет использовать их в качестве предикторов успешности адаптации.

Исследование социальной адаптации групп юношей ШТР с разным катамнестическим выходом

Характеристика группы с хорошим катамнестическим выходом — группы ремиссии (ГР). Пациенты данной группы пришли на повторное обследование по просьбе исследователей. Возраст на момент первого обследования в подростковом возрасте 12—15 лет, повторное исследование проводилось в 19—23 года. В юношеском возрасте 4 человека учились или уже окончили средне-специальное или высшее учебное заведение. Среди специальностей, которыми овладевали пациенты группы ремиссии в юношеском возрасте, были: юрист, менеджер, инженер, филолог, цветовод.

При первом обследовании у пациентов отмечались проблемы со школьной успеваемостью, нарушение взаимоотношений с окружающими-сверстниками, родителями, учителями, утомляемость, депрессивное состояние, повышенная тревожность. Большинство испытуемых в подростковом возрасте имели заниженную самооценку, негативно характеризовали себя, отвечая на вопросы методики «Кто я», «Какой Я».

В юношеском возрасте у респондентов данной группы самооценка стала более адекватной, более высокой стала мотивация. Пациенты старались занимать активную социальную позицию, практически все имели друзей, двое состояли в сообществах по интересам, двое задумывались о создании семьи.

Таким образом, социальную адаптацию этой группы можно оценить положительно, по большинству показателей данная группа ближе к контрольной выборке. Пациенты данной группы показали при повторном исследовании положительную динамику по показателям слухоречевой памяти (в частности, по непосредственному воспроизведению), по уровню обобщения, при сохранившихся некоторых особенностях эмоционально-личностной сферы. Можно говорить о стабилизации процесса и об улучшении социальной адаптации.

Характеристика группы с плохим катамнестическим выходом — группы с текущим заболеванием (ТЗ). Респонденты группы с текущим заболеванием повторно стационировались в юношеском возрасте в НЦПЗ РАМН. В этой группе первое обследование проходило в 12—15 лет, на момент последнего обследования их возраст 17—26 лет. В юношеском возрасте учились 4 человека — из них один в школе на домашнем обучении, двое на заочном отделении коммерческого вуза по специальности менеджмент, один на вечернем отделении в коммерческом вузе. На момент обследования все они находились в академическом отпуске. Двое других респондентов получили образование: один окончил колледж, но не работает, другой пациент окончил МАИ платное отделение, работал, но уволился в связи с конфликтами на работе. Последний испытуемый окончил школу, но затем не работал и не учился. При первом обследовании в данной группе выявлялась неадекватная самооценка, высокие уровень тревожности, негативный эмоциональный фон при распознании жестов и поз, агрессивные и фобические тенденции. Повторное обследование группы пациентов с текущим заболеванием выявило снижение самооценки и мотивации, усиление чувства вины и обиды с возрастом. Почти половина пациентов этой группы, несмотря на полученное образование, не хотели работать, планировали основное время проводить дома, т.е. динамика их социальной адаптации была отрицательной.

Обсуждение

Таким образом, проведенное исследование показало, что юношеская группа пациентов с ШТР демонстрирует лишь небольшие нарушения когнитивного функционирования по сравнению с контрольной группой здоровых испытуемых, получивших или получающих высшее образование в одном из лучших высшем учебном заведении страны. При этом выявленные особенности памяти и внимания в подростковом возрасте не явились предикторами будущего нарушения социальной адаптации. С возрастом отмечалось улучшение такого показателя памяти, как «непосредственное воспроизведение» в группе ремиссии. Ранее нами было показано, что этот параметр тесно коррелирует с определенными личностными особенностями (Горбачевская, Кобзова [7]), которые заметно нивелировались у пациентов этой группы в юношеском возрасте. В работе Хромова [16] было показано, что прогресс по показателям памяти в юношеском возрасте характерен для группы ШТР. Не явились предикторами успешного выхода и показатели внимания, которые не улучшились с возрастом и не отличались в двух группах ГР и ТЗ ни в подростковом, ни в юношеском возрасте. Использованные в данной работе характеристики мышления также не показали существенных различий между группами ГР и ТЗ в подростковом возрасте и также не могут помочь в определении дальнейших адаптационных возможностей.

По всей вероятности, у пациентов с ШТР основную роль в успешности социальной адаптации играет высокая мотивация, саморегуляция, в то время как затруднение установления социальных контактов приводят к проблемам с получением образования, работой по специальности, самостоятельностью, созданием семьи, что было показано на примере группы пациентов с ТЗ. Сходные данные отмечены в работах Борисовой и др., [1; 12], где было показано, что пациенты с шизотипическим и шизоидным расстройством адаптированной группы устанавливали положительные, но формальные контакты с окружающими, проявляли интерес к учебе и работе по специальности, для пациентов дезодаптированной группы с теми же диагнозами был характерен низкий уровень социальной адаптации, отсутствие занятости, самоизоляция, трудности планирования будущего. Предикторами дезадаптированности являлись низкий энергетический потенциал, психический инфантилизм. Таким образом, по-видимому, именно личностные особенности и негативная симптоматика играют основную роль в социальной дезадаптации пациентов с ШТР, тогда как когнитивные нарушения в юношеском, и особенно подростковом возрасте, не могут быть использованы в качестве коррелята адаптационных возможностей.

Abstract. The goal of research is studying the possible influence of cognitive violations in adolescents and young persons with STD on their social adaptation. Materials and methods. The main experimental group included young persons with schizotypal disorder, which undergo inpatient or outpatient observation in the Mental Health Research Centre. There were 33 persons at the age of 17—28, the mean age being 22. 12 out of 33 patients had been examined in the Mental Health Research Centre starting from the adolescent age (12—15 years), catamnesis from 3 to 12 years; among them the remission group and group with unfavourable disease course were allocated. The control group included healthy subjects, selected with the help of SPQ Questionnaire, 20 persons, male MSU students and graduates at the age of 17—28 years, the mean age is 20 years. The following methods have been used: Memorizing 10 Words, Schulte Tables, Small Subject Classification, Object Designing and Scale Self-Assessment (Dembo-Rubinstein). Clinical interview, examining medical records and talks with relatives have been applied as well. Research results and discussion. Comparison between group of young persons with STD and control group have shown that young persons with STD have lower socioeconomic status (they have more study leaves, 21% of test persons have moved to less prestigious institutes, 36% have neither studied nor worked). Research results have showed that cognitive data in the groups of remission and current disease in adolescent age are almost identical, therefore they do not serve as predictors of successful adaptation in youth period. In youth age the positive dynamics on memory data in both groups has been marked. It is important to note that some data in the remission group reached normative level (they memorized on the average 7 words at the direct recall). The attention data were almost identical to those in adolescent age; in youth age, the attention data didn’t have positive dynamics, therefore patients found numbers significantly more slowly (Р≤0.01) in comparison with control group. Thinking data (generalization level) in the remission group of youth age had positive dynamics, at that thinking data in general did not differ significantly the whole STD group from control group, therefore only memory data in youth age are more clear-cut predictors of further prognosis. Social status analysis have shown that adaptation of the remission group is much higher that in the group of current disease. Patients in the remission group tried to take an active social position, almost each patient had friends, worked or studied at the university. Almost half of the patients in the current disease group did not work, though they had received degree, they were going to spend most of their time at home, i.e. their social adaptation dynamics was negative. Conclusion. Thus, the research have showed that it is personal qualities (low self-esteem, reduced energy potential, affective flattening, volitional violations and social isolation), which play key role in social maladaptation of patients with STD, while cognitive violations in youth age, and especially in adolescent age, can not be used as clear-cut correlates of adaptation capabilities.

Introduction

Schizotypal disorder is quite a widespread mental disease; thus, the American Psychiatric Association says that its incidence reaches 3—6% [17; 18]. In our earlier research Kobzova and others [9; 10], a high risk of STD development has been revealed in 3% cases in the cohort of healthy test persons. Despite the ambiguous interpretation of this disease [2; 15; 27], Russian and Western researches have shown that schizotypal disorder can be accompanied by cognitive and personal impairments as well as social maladaptation [1; 5; 6; 11; 12; 13; 14; 21; 23; 24; 26; 35] and is mostly associated with negative symptoms [3; 19]. Besides, persons with schizotypal disorder are relatives of the first degree of persons with schizophrenia in 50% cases [28]. It should be noted that deficit in cognitive and personal sphere in persons with STD can be a predictor of schizophrenia [20]. Therefore, studying the dynamics of various aspects of functioning of persons with schizophrenia-related disorders as well as revealing the predictors of these diseases is an important task. The dynamics of clinical and psychological peculiarities in patients with schizophrenia-related diseases and analysis of dynamics types (therapeutic, katamnestic and age-related) have been studied by such authors as V.G. Kaleda [8], V.P. Kritskaya [12], T.K. Meleshko [13], D.Yu. Borisova [1], A.I. Khromov [16],  A. Raine [22; 33], Ledda [25].

Study of memory indices and their dynamics. V.P. Kritskaya and T.D. Savina [11] have shown in their work that schizophrenic process is accompanied by memory impairments — change of mnestic activity structure that grows as the defect becomes more severe. Heaton and others [31] registered memorization deficit at the beginning of the task in patients with STD, however, delayed reproduction of information did not differ from the control group (norm) Bergman and others [35]; other researchers register the impairments of prospective memory (intention memory) in persons with schizotypal disorder [32].

A.I. Khromov [16] has studied the dynamics of direct audio-verbal memory in children (7—8 years) and adolescents (14—16 years) in the groups of persons with schizophrenia, schizotypal disorder and healthy persons. It has been shown that retardation from norm according to the volume of direct reproduction ends in 15—16 years in the group of patients with STD. They have shown an increase of memory volume up to the age norm.

Study of attention indices and their dynamics. The authors highlight deficit of information receipt and processing [29], inability of persons with schizophrenia to single out the main stimulus [30], active attention impairment associated with affective sphere and motivation as well as passive (selective) attention impairment in persons with schizophrenia-related disorders. Patients with STD were also characterized by a longer cross-modal transfer from visual to audial stimulus [36]. A.I. Khromov [16] has shown that attention changes associated with age in persons with schizophrenia-related disorders as well as in persons from norm group, demonstrate clear positive dynamics; retardation from norm in f 21 group, which is severe at the age of 7—14, levels by 15—16 years.

Study of social maladaptation and its dynamics. Katamnestic study of adolescents diagnosed with schizophrenia, schizoaffective psychosis and bipolar disorder (5 years) conducted by Ledda and others [25] has shown a worse dynamics of social and cognitive parameters at the age of 20—25 in the group of patients with schizoaffective psychosis and schizophrenia than in patients with affective diseases.

Thus, a short review of domestic and foreign works devoted to the issue of our research has shown that the study of dynamics in cases of schizophrenia-related diseases is an important task for medical psychologists, clinicians and psychopsysiologists. It should be noted that there is but a few works devoted to katamnestic studies of patients with schizotypal disorders at the age interval of 12—28. Therefore, the goal of our research is studying the possible influence of cognitive impairments in adolescent and youth patients with STD on their social adaptation.

Materials and methods

Adolescents and young persons with schizotypal disorder (F21.х according to IDC-10) as well as healthy test persons aged from 17 to 28 took part in the study. All the healthy test persons (20 MSU students and graduates, the mean age is 20 years) were selected on the ground of testing by means of SPQ (Schizotypal Personality Questionnaire) (А. Rainе [33], A.G. Efremov, S.N. Enikolopov [4]). Only those test persons whose composite score on this questionnaire was lower that 23 were included in that group. Criteria of entering the clinical group: age from 17 to 28 years, male sex, "schizotypal disorder" diagnosis, disease onset in the adolescent age. The main experimental group of STD patients (33 persons at the age of 17—28, the mean age being 22) underwent either inpatient or outpatient observation in the RAMS Mental Health Research Centre. Each patient underwent psychoactive therapy and was examined in the state of outcome in remission. 12 out of 33 patients had been examined in the RAMS Mental Health Research Centre starting from the adolescent age (12—15 years), catamnesis from 3 to 12 years. 5 persons were in a long-lasting remission (remission group (RG) while the other group (7 persons) was hospitalized to the RAMS Mental Health Research Centre repeatedly at the adolescent age (current disease group (CD).

Studying of cognitive functions Memory and attention indexes were studied by means of "10 Words Memorizing" and "Schulte Tables" techniques; auditory and verbal memory volume (the number of memorized words) as well as attention volume (time, spent for finding numbers in Schulte tables) were analysed. Thinking was studied by means of "Small Subject Classification" and "Object Constructing" tests; such indexes as generalisation level (test person’s ability to use categories), impairments of thinking (group classification based on specific attributes accompanied by distorted generalisation as well as creation of situations and complexes), commonality coefficient (it is evaluated whether test persons perform a task conventionally or unconventionally as well as what common characteristics of subjects form basis for their classifications) and productivity (number of fulfilled tasks) were compared between groups. Besides, the self-esteem of patients and test persons from the control group was studied by means of "Scale Self-Esteem" technique (Dembo-Rubinstein) according to "health", "intelligence", "happiness" and "temper" parameters.

Social adaptation was studied by means of clinical interview, medical records examination and interviews with relatives.

Statistical analysis of the results was performed by means of Exel and Statistica 7 — the MS Office Excel application. We applied and analysed additional pathopsychological and personality questionnaires in individual cases to assess the results, obtained in the catamnestic groups due to the small number of patients there. We used quantitative analysis of the averages and qualitative analysis. Student’s T-test was applied to compare the results of general STD and control groups, differences were significant at Р<0.05.

Results

Comparative analysis of social and economical status of patients with STD and control group in youth age

Social status of experimental group was lower than that of control group according to the majority of the parameters. Patients with STD took academic leaves more often, 21% of them moved into less prestigious colleges, 36% of patients were neither working nor studying during the examination. Young persons from main experimental group lived with their parents, while 30% of young persons from control groups lived independently. The analysis of the study results of two catamnestic groups: remission group (RG) and current disease group (CD) has shown that the level of social adaptation in the remission group is much higher than in the current disease group, because the majority of RG young persons either worked or both worked and studied, while none of the CD group worked, despite the obtained degree.

We carried out a comparative study of memory and attention indexes in youth patients with STD and control group and analysed those indexes separately in patients from RG and CDG in adolescent and youth age in order to find connection between successful social adaptation and cognitive impairments. The best memory and attention indexes were found in the control group. They had better indexes of "Memorizing 10 Words" technique than the STD group; however significant differences were obtained only for the parameters of "direct reproduction" (Р<0.04) and "second reproduction" (Р<0.03).

It was interesting to find out whether this index was a predictor of bad outcome in the adolescent age. It turned out that during the first examination the 12—15 aged adolescents from RG reproduced less words than those from CD group, i.e. this index cannot determine the quality of outcome. Moreover, the delayed reproduction in youths from CD group was better than that in the remission group.

Thus, the positive dynamics of memory indexes, in particular, the direct reproduction in the remission group was revealed. However as the research has shown, memory indexes in the adolescent age do not appear to be the predictors of successful social adaptation.

Comparative study of attention indexes in patients with STD of youth age and control group. The technique of finding numbers according to Schulte tables was applied for studying attention. The study of attention has shown that the norm group finds the numbers according to all the Schulte tables quicker (33—35 sec. per table) in average than the group of patients with STD 40—50 sec. per table). The results are significantly different (p≤0.01). The comparison of average attention indexes in Schulte tables in CD and RG groups in adolescent and youth age and youths from control group are represented in the pic. 2.

The represented bar charts show that the adolescents from CD group find numbers quicker than adolescents from the RG group, though no significant differences were revealed. The time of finding numbers increases to the third table in the adolescent groups, which can be regarded as the reflection of fatigue process. This index did not change significantly in patients with STD in youth age. The results of both groups differ from the control group, the patients find numbers slower (minimal search time in youth age is 39 sec., maximal — 47.6 sec.). In the control group (minimal search time is 34.3 sec. and maximal time of searching numbers is 36.7 sec.).

It can be said in general that patients with STD demonstrated worse attention indexes than the test persons from the control group, at that no distinct age dynamics of attention indexes in both groups (remission and current disease) was observed and no difference between these indexes in RG and CD patients was found. Thus, the attention indexes did not appear to be the predictors of successful social adaptation either.

Comparative study of thinking in patients with STD and control group. Comparison of thinking parameters: "generalisation level" (GE) and "thinking impairments" (TI) in the group of patients with STD and control group has shown that generalisation level in STD group (58%) is almost identical to that in control group (59%). Assessing the age dynamics in two groups according to generalisation index, one can note positive dynamics in the remission group and its absence in the CD group. Thus, the "generalisation level" index in the adolescent age can not predict further state dynamics.

The level of thinking impairments in young persons appeared to be lower in the STD group (29%) than in the group of norm (39%), however no significant differences were revealed.

Comparison of STD group with control group in the "Object Constructing" test. Comparison of standard coefficients in the "Object Constructing" test in the group of patients with STD in youth age with control group has shown that standard coefficient is higher in the control group (0.63) than in the STD group (0.58); however these data do not differ significantly. In addition to the standard coefficient, we compared the groups on productivity. Productivity in both groups is the same, 0.88 и 0.85 points.

Thus, thinking indexes have revealed no significant differences between the STD groups and control group, which also does not allow to use them as the predictors of successful adaptation.

Study of social adaptation in the groups of young men with STD with different catamnestic outcome

Characteristic of the group with good catamnestic outcome-remission group (RG). Patients of that group came to be examined repeatedly at the request of researchers. The age at the moment of the first examination in the adolescent age was 12—15 years, the repeated examination was carried out at the age of 19—23. Four persons of the youth age either have been studying or have already graduated from college or university. The patients of youth age in the remission group had the following specialities: lawyer, manager, engineer, philologist and florist.

At the first examination the patients were marked by the poor school results, impaired relationships with peers around them, parents and teachers, fatigue, depressive state, increased anxiety. The majority of test persons had low self-esteem in the adolescent age and characterized themselves negatively while answering the questions of the "Who am I" and "What am I" technique.

The respondents of that group acquired more adequate self-esteem and higher motivation in the youth age. Patients tried to take an active social position, almost each patient had friends, two patients were the members of communities of practice, another two patients were considering starting a family.

Thus, social adaptation of this group can be evaluated positively; the given group is closer to the control sample according the majority of indexes. Patients of this group showed positive dynamics according to the indexes of auditory and verbal memory (in particular, the direct reproduction) and generalisation level at the repeated examination, though some specific features of emotional and personality sphere were preserved. We can speak about the process stabilisation and improved social adaptation.

Characteristic of the current disease group (CD) with a bad catamnestic outcome. Respondents of the current disease group were hospitalized repeatedly in a youth age in RAMS Mental Health Research Centre. Patients of that group were first examined at the age of 12—15 years, their age was 17—26 years at the moment of the last examination. Four persons studied in the youth age: one of them was home-schooled, two studied at the extramural department of the commercial institute majoring in management, another one — at the evening department of the commercial institute. All of them had the academic leave at the moment of examination. Another two respondents got a degree: one graduated from the college but did not work, another graduated the MAI paid department, worked, but quitted the job because of the conflicts with colleagues. The last test person finished the school but than neither worked nor studied. That group demonstrated inadequate self-esteem, high level of anxiety, negative emotional background at the gesture and pose recognition as well as aggressive and phobic tendencies at the first examination. The repeated examination of the group of patients with current disease revealed the reduce of self-esteem and motivation and the increase of feeling of guilt and offence with age. Almost half of the patients of that group did not want to work despite the obtained degree; they were going to spend most of their time at home, i.e. the dynamics of their social adaptation was negative.

Discussion

Thus, the study has shown that the youth group of patients with STD demonstrates only slight impairments of cognitive functioning in comparison with the control group of healthy test persons, who has graduated from or are studying at the best university of Russia. Besides, the revealed memory and attention specific features in the adolescent age did not appear to be the predictors of future impairments of social adaptation. It was noted that such memory index as "direct reproduction" improved with age in the remission group. We have shown earlier that this parameter closely correlates with specific personality characteristics (Gorbachevskaya, Kobzova [7]), which notably evened in the patients of that group in the youth age. Khromov [16] has shown in his study that the progress according to the memory indexes in the youth age is specific for the group with STD.

Attention indexes, which neither improved with age nor differed in both RG and CD groups in neither adolescent nor youth age, did not appear to be the predictors of successful outcome either. The characteristics of thinking that were used in this study have not shown significant differences between RG and CD groups in the adolescent age either and are not helpful in defining further adaptive capabilities as well.

It seems probable that high motivation and self-regulation play a key role in the successful social adaptation of patients with STD, while difficulty in making social contacts leads to the problems with getting a degree, working in one’s degree field, independence and starting a family as it was shown on the example of the group of patients with STD. Borisova and others [1; 12] noted similar data in their works and showed that patients of the adapted group with schizotypal and schizoid disorder made positive though formal contacts with people around them and demonstrated interest in studying and working in their degree field; patients of maladapted group with the same diagnoses were characterized by the law level of social adaptation, inoccupation, self-isolation and difficulties of future planning. Law energy potential and psychic infantilism appeared to be the predictors of maladaptation. Thus, it is probably personality qualities and negative symptoms that play a key role in social maladaptation of patients with STD, while cognitive violations in youth age, and especially in adolescent age, cannot be used as a correlate of adaptive capabilities.
